Compliance Incident	Any incident involving general compliance, legal, regulatory, ethical, other matters of non-compliance, e.g. – fraud, coding and billing errors, violations of laws or organizational policies, Code of Conduct violations, retaliation, bribery, conflict of interest.
Privacy Incident	Any incident involving the possible improper access or disclosure of patient protected health information (PHI), e.g. – documentation errors, inappropriate medical record access, registration of the wrong patient, paperwork given to wrong patient, posting of PHI on social media.
